Abstract: 

The term “MILITARY DENTISTRY” always stirs people's curiosity, whileMILITARY DENTISTRY does not differ much from the discipline of DENTISTRY. This field is the same as the science of DENTISTRY that places in specific situations and locations of MILITARY. So, for the welfare of the troops, it should have certain characteristics that are coordinated with the purposes of this group of society. Type of patients, equipment and planning for education in this special group are different from other segments of society. Also, this field requires certain individuals as MILITARY dentists, and people also teach them should have special conditions In this review, it has been tried to clarify the importance of MILITARY DENTISTRY. In our country, in order to develop this important branch of the discipline of MILITARY medicine, the cooperation of all professionals and researchers in this field is mandatory.

مقدمه: در علم بر پایه مدارک (Evidence-based) پزشکان و دندانپزشکان بر اساس شواهد و مدارکی که از منابع مختلف به دست می آورند برای بیماران تصمیم گیری می کنند، ولی آیا هر مدرکی با هر درجه از اعتبار می تواند مبنای صحیح و درستی برای تصمیم گیری باشد؟ طبق مدارک موجود بیش از 30 درصد از دستورالعمل های درمانی و مدارکی که از کتاب ها و مراجع به دست می آیند اشتباه بوده، گمراه کننده می باشند.در منابع دیگر نیز آمار و ارقام تقریبا مشابهی وجود دارد. درصد بالایی از این دستورالعمل های درمانی اشتباه ماحصل فقدان ها و کمبودهای موجود در تحقیقات بالینی است، که از علل متعددی همچون عدم اطلاع محققین از مبانی پژوهش، محدودیت های پژوهش و تحقیق، ارایه غیرواضح مطالب و بیان نتایج کاذب منشا می گیرد. امروزه برای شناخت نقاط ضعف و قوت مدارک و شواهد، علم پزشکی مبتنی بر مدرک (EBM) یا دندانپزشکی مبتنی بر مدرک (EBD) شکل گرفته است.

Dear Editor, MILITARY DENTISTRY is a strange and unknown field for the general public. MILITARY DENTISTRY is one of the branches of DENTISTRY that has very special differences from other branches of DENTISTRY. Considering the challenging nature of DENTISTRY, perhaps MILITARY DENTISTRY can be regarded as one of the most challenging fields of medical sciences. Due to the lack of coherent scientific articles in this field, in this article, we tried to discuss the history of MILITARY DENTISTRY and the formation of dental units in the armies. The history of dental problems goes back to the dawn of human life on earth, and the history of quarrels and the need to defend goes even further back. Therefore, presumably, the first traces of DENTISTRY date back to such old times that it is impossible to track down the precise historical traces of DENTISTRY...

Abstract: 

Background and Aim: According to the clinical approach of standards of oral health system, designing specific standards is necessary to guarantee health promotion activities in the dental clinic. The aim of this study was to develop the standards of the health promotion dental center and to evaluate the performance status of a MILITARY dental clinic based on the standards. Methods: An applied descriptive study of consecutive (qualitative and quantitative) method was conducted in 2019. At the qualitative stage, the standards were collected and analyzed through library study and Delphi method. To obtain expert opinions, the draft of each standard axis including, title and standard definition, target expression; definition of sub-standards and criteria was provided to 11 participants through a Delphi questionnaire. In the quantitative phase for evaluating the studied center, the data were collected using the Health Promotion Dental Standards Checklist tool derived from the qualitative phase. The checklists were completed by the assessment team at the study center and their data were analyzed using SPSS-16 software. Results: The Health Promotion DENTISTRY Standards consisted of 6 axes of management policy, patient evaluation, patient notices and interventions, promoting a healthy workplace, continuity and collaboration, and 61 criteria were approved by experts by the content and standards of the two Delphi rounds. The quantitative results of the standards assessment were evaluated on the medium level. Among the standards, the highest score was for management policy evaluation, 68. 62% of the total score. Patient evaluation was scored 66. 66% on the axis, followed by patient notices and interventions scored 52. 38% and continuity in treatment and participation scored 48. 80% of the total score. The lowest score was 41. 66% on promoting a healthy workplace. Conclusion: Health promoting standards are necessary for dental centers with a core health mission and to establish a mechanism for evaluating their performance in implementing health promotion programs. In order to improve the quality of oral health services, it is recommended that policymakers and managers of dental centers be effective in improving health-based dental services by establishing coherent policies within the framework of health promotion standards.

Abstract: 

Aims: Digital DENTISTRY is not the wave of the future, it is happening now. Whether a dentist embraces the new technology determines the quality of treatment and possibly his future,for this reason, dentists and medical centers have no choice but to use and become familiar with digital DENTISTRY. The present study aimed to investigate the cost-effectiveness of setting up a digital dental laboratory for a dental clinic and ultimately use its results to make better decisions about setting up this department. Materials and Methods: The present cross-sectional-analytical study was conducted with the participation of 10 dental laboratory technicians and dental equipment stores, as well as 108 dentists and other dental department staff of several MILITARY centers in Tehran in 2024. In the first stage, a briefing plan for a prosthetic department with a digital approach was developed. Then, a questionnaire was developed based on the study of Bowen et al. to assess the opinions of dentists and dental department officials, and its validity and reliability were evaluated by 10 general dentists or dental prostheses specialists. Then, after reading the summary of the briefing plan, the participants answered the questions of the standardized questionnaire. Data were analyzed using SPSS 27. 0. 01 software and independent t-test. Before performing statistical tests, the assumption of normality of the total score distribution was examined using Shapiro-Wilk and Kolmogorov-Smirnov tests. Findings: In the prepared justification plan, the investment cost required to launch a digital dental department was estimated at 10 billion Tomans, and then profitability indicators were calculated, and all indicators were assessed as appropriate (accumulated profit and loss of the first to fourth year in terms of million Rials, 16, 096 and 58, 059 and 140, 239 and 276, 621, respectively, as well as an internal rate of return of 60 percent and a payback period of 3 years). In the survey, participants predicted a high need for this department (76. 8 percent of people) and a high use of the services provided by dentists in this center (86. 4 percent). On the other hand, based on the participant's perspective, technical ability (65. 7 percent of people), the ability to provide the necessary environment (60. 9 percent of people), and the ability to provide financial resources for the initial investment (52. 8 percent of people) were the limitations in launching a digital dental department, respectively. Conclusion: Based on the developed plan, a review of service statistics, and then calculating the cost-income ratio, financial indicators assess the appropriateness of establishing such a center.

Abstract: 

Background and Aim: MILITARY DENTISTRY combines patient-centered care with public health views to create services agile and comprehensive. The effect of oral disease on MILITARY personnel can significantly damage the individual's operational ability. A significant concern of various countries in providing universal health coverage is access to dental services and oral health care, which requires understanding the related barriers and facilitators. This study aimed to determine systematic relationships among determinants of access to preventive dental services. Methods: This mixed-methods, descriptive-analytical study was carried out in 2021 using an operations research technique. In the qualitative phase, by the literature review and using the snowball method, an interview was done with ten experts in dental prevention and oral health. The conceptual research model based on the Saurman access model was designed into 30 sub-components and six main components by identifying and categorizing the obtained codes. In the quantitative stage, a 30-item questionnaire was designed using the Decision Making Trial and Evaluation Laboratory (DEMATEL) technique. The questionnaire was completed by 30 experts selected from medical sciences staff and line using purposive sampling. The data were analyzed in SPSS and DEMATEL through structural analysis of causal relationships. Results: A total of 30 variables were identified based on the qualitative findings. These variables were categorized into six components, including affordability, acceptability, accessibility, awareness and empowerment, adequacy, and availability based on the analytical framework of the study. The experts confirmed the proposed determinants of access to dental services and suggested no other component (P = 0. 0001). Analytical results indicated that "affordability" (impact factor = 1. 05) and "acceptability" (impact factor = 0. 37) affected access and were therefore assigned to the causes group. However, empowerment, accessibility, adequacy, and availability were influenced by access and were therefore assigned to the effects group. Finally, affordability (1. 05, 11. 88) and availability (-0. 57 and 11. 35) were identified as the most and most miniature essential components, respectively. Conclusion: An essential factor in MILITARY personnel is "hidden" dental complications. By evaluating dental complications, this component is reduced approximately one-third. Identifying critical factors of access help design prevention interventions, health prevention, and promotion policies and removing barriers to public access to oral health care.

Abstract: 

Background and Objective: The main aim in medical education is teaching students to carry out certain duties in future. The Restorative DENTISTRY is a very important dental practice in our country. The aim of this study was to determine General DENTISTRY restorative curriculum's adaptation to restorative DENTISTRY service needs in community.Materials and Methods: In this quantitative cross‐sectional study consisting of two phases, data was gathered by two valid and reliable checklists. In first phase, the restorative dental services of 1027 patients in a dental clinic sampled over a period of three months were analyzed by statistical software SPSS.17 and the results were reported. In the second phase, we provided a list of syllabuses and restorative lessons with summarized results of the first phase to Tehran and Shahed University restorative faculty members and asked them to express their views about General DENTISTRY restorative curriculum's adaptation to the analyzed needs in last phase and results were reported.Results: In first phase, 2362 cases of the restorative service were considered. The results showed that all the syllabuses of restorative curriculum met the restorative needs of the patients. But the amount of time allocated to teaching “theoretical cardiology” and “practical restorative DENTISTRY3” did not meet the students' needs.Conclusion: Restorative lessons syllabus was suitable but time was not sufficient. So the amount of time of teaching should be added to fix the deficiency, so that the quality of education can be promoted. For this purpose using new methods of education via educational package, films and modules may be useful.

Abstract: 

A B S T R A C T Transformations such as globalization, the contiguity of geographical spaces, as well as developments in the field of MILITARY technologies have transformed the issue of defense as the main factor for the survival of countries. Because the transformation from hard to soft war and then the smart war in recent decades has changed the approaches to defense. These transformations have led to the development of discussions about defense purposes. Meanwhile, Middle East countries have a prominent position and importance in terms of defense due to their special conditions. This article has tried to investigate the basic dimensions affecting the defense of foreign countries with descriptive and analytical methods, using library and field findings and Smart-PLS software. The results of the current research show that the effective components in the current research include 130 items that are placed in the form of 15 dimensions. The importance of these dimensions in defense of the Middle East countries is, respectively, geopolitical dimension with a 0.43 score, MILITARY dimension with a 0.41 score, economic dimension with a 0.41 score, political dimension with a 0.39 score, hydrology dimension with a 0.34 score, security dimension with a 0.32 score, social dimension with a 0.29 score, demographic dimension with a 0.28 score, cultural dimension with a 0.26 score, scientific-health dimension with a 0.25 score, geomorphological dimension with a 0.25 score, mathematical dimension with a 0.2 score, climatic dimension with a 0.2 score, biological dimension with a 0.2 score and soil dimension with a 0.18 score. Extended Abstract Introduction Security and defense are among the most vital issues for the preservation and survival of countries. In fact, defense and MILITARY affairs are necessary for countries' independence and political, cultural and economic development. Among them, geography and political geography are prominent fields that directly affect defense issues. Actions taken by a country in dimensions (political, MILITARY, economic, etc.) in order to maintain security and territorial integrity, guarantee independence and protection of its people against any enemy attack, will organize the geographical (spatial) environment, wise and appropriate distribution of critical places and sensitive centers and infrastructures are identified by taking into account the threats and the correct use of environmental capacities in political, economic, social, cultural, etc. dimensions, and as a result, it can lead to stability and stability in the country's geographical space. The design of defense planning model according to the economic, cultural, political and environmental features in the new era when countries are facing external and internal threats as a sub-branch of territorial planning has received the attention and importance of officials and governance systems in order to be able to reduce these threats with accurate and systematic planning in the geographical space, defense planning is not separate from the flow of national and regional planning. It is important to pay attention to it based on the type and amount of external and internal threats to governance. Due to the expansion of the security circle, the government is not only responsible for creating MILITARY security. However, it must also exercise part of its authority in the direction of territorial management. On the other hand, there has been a transformation in the form of threats and wars based on changes and the movement from hard to soft war and finally, smart war is the basis for reviewing and recognizing the components that can be used in defense of countries, including the Middle East countries, have a prominent position and importance. Despite the internal differences between countries, the Middle East has commonalities and connections that can directly affect the overall defense of this geographical space.   Methodology In terms of type and purpose, the current research is fundamentally applied. Also, it is descriptive and analytical in terms of nature. This research, in addition to the correct and realistic depiction of the defense situation of the Middle East countries, it is tried to provide a suitable model for the defense of the countries. The sources used in the current research are library and field sources (questionnaires). According to the subject of the current research, the statistical population of this research is a collection of people, experts and elites who have sufficient knowledge, experience and expertise regarding the research topic, which is the design of the defense model, according to the nature of the Delphi pattern model, at least the statistical population based on the sources should be between twenty and fifty people to reach statistical saturation. The number of selected samples included 51 people who were identified in the form of a non-random sampling pattern and introduced as a statistical sample in the framework of the Delphi model. Due to the qualitative nature of this research, it has been tried to use the Delphi model in the framework of the targeted non-random model. Therefore, based on these cases, the current research has selected the maximum statistical population of 51 elites, experts, and opinionated people in this field. In this research, to evaluate the research hypotheses in the form of the hardness equations model, the partial least squares method and the SMART-PLS software, which is a variance-based path modeling technique and provides the possibility of checking the theory and metrics simultaneously, have been exerted.   Results and discussion In this research, approaches such as political, MILITARY, cultural, etc., in the field of land use have been discussed. In the meantime, the defense of countries as an emerging approach is significant. According to the developments in geographical spaces, such as the evolution of MILITARY-defense technologies, the connection of geographical spaces to each other and most importantly, the movement from hard and soft war to smart war has witnessed a transformation and wide attention in a way that countries are suited to these developments are trying to prepare the geographical space of their country in terms of defense. Therefore, the defense planning of the Middle East countries as a geographical region has great importance from a geopolitical and strategic point of view, which indicates that the countries of this region face threats and complex defense-security issues in different ways.   Conclusion The results of the present research show that the defense planning in developing countries should be considered according to their characteristics, considering all 15 dimensions, which include 130 components. In the design of the defense planning of these countries, attention should be paid to the weight and importance of the shaping dimensions. In general, it can be said that the defense planning of the Middle East countries is proportional to the geographical features of these countries and on the other hand, considering the developments in the field of threats and moving from hard to smart war, 15 dimensions should be considered for the defense of these countries.   Funding There is no funding support.   Authors’ Contribution Authors contributed equally to the conceptualization and writing of the article.
